Surgery Patient Information

If you are a surgery patient or anticipating a hospital admission, you will be scheduled a time to come for your routine preoperative tests, usually one to two weeks before admission.

When you arrive, go directly to Patient Registration, located on the first floor lobby area. Patient Registration staff will finalize the details of your admission with you.

What to Bring:

  • A list of your prescribed medications
  • A list of your allergies
  • Your current health insurance information

Advance Directives

When you talk with the patient representative, you will be asked if you have an Advanced Directive, such as a living will or other instructions for your care if you are no longer able to decide for yourself. Current law requires that we ask this question and it is not cause for alarm. If you would like more information about Advance Directives we will be happy to help you and provide forms. You may also download information from this web site. If you already have an Advance Directive and we do not have a copy on file at our hospital, please bring a copy with you.

Insurance Pre-certification

Many insurance companies require pre-certification for a planned hospital stay. The Patient Representative will remind you to call your insurance company before you come to the hospital to get approval prior to your admission.

Admission Day

You will be notified of the time you will need to come to the hospital one day before your admission. Please bring protective containers for eyeglasses, contact lenses and dentures. Please do not bring large sums of money or other valuables.
